Using the service
FormSlam converts documents into editable forms and lets you save or export submissions. You must be authorized to upload, process and share the content you provide. Keep your credentials private and do not use the service to distribute malicious content, bypass usage limits or access another user's records.
Review generated output
OCR, generated forms and company suggestions may be incomplete or incorrect. You are responsible for reviewing labels, fields, validation rules, suggested values and source links before relying on the result. The service does not certify documents or provide legal, financial or compliance advice.
Credits and availability
One credit is reserved for each accepted form-processing job, consumed on success and returned on terminal failure or cancellation. Administrators manage credit allowances. This release has no self-service payments, subscriptions or checkout. Processing is subject to configured limits and provider availability. Keep exported copies of records you need.
Your content and account
You retain ownership of your content and permit its processing by FormSlam and the providers described in Privacy to deliver the requested service. You can delete templates, submissions and your account. Deletion and retention behavior are described in Privacy and Help. Accounts may be restricted for abuse or attempts to compromise the service.
